How steep are Lisbon's hills really? Is walking between neighborhoods realistic?

Alfama and Bairro Alto/Chiado are the steepest zones, narrow, winding streets with real grade to them, not gentle inclines. São Jorge Castle sits at the top of one of the steepest climbs in the city, and Bairro Alto has enough of a rise that most people are noticeably out of breath by the top. Add old, uneven cobblestone paving and it's genuinely tougher walking than a flat city with the same elevation numbers would be.

That said, it's pocket-dependent, not uniformly brutal everywhere:

Baixa (downtown) — flat, easy grid, no issue walking this at all.

Belém — flat along the waterfront, easy walking once you're there, the issue is just that it's far from the center.

Alfama, Bairro Alto, Graça — steep, cobblestone, genuinely tiring, especially in heat.

So realistically: walking between neighborhoods works fine if you're going flat-to-flat (Baixa to Belém area, once you're out there) but gets rough fast if you're doing hilly-to-hilly (Alfama to Bairro Alto, for example) back to back, especially with cobblestones adding ankle strain most people don't expect.

This is exactly why so many people end up doing hop on hop off Lisbon for the steep or far-flung connections (Belém especially) and saving their own legs for wandering the flat or moderately hilly core on foot. Tram 28 is the other cla###ic workaround since it's built specifically to handle Alfama's steepest bits, but it's crowded enough that a lot of people find the HOHO bus a calmer alternative for the same climbing problem.

If you've got decent stamina and don't mind slow, careful walking on uneven stone, it's doable without leaning on transit much. But if you're already packing a lot into one day, the hills plus cobblestones will slow you down more than flat-city mileage would, worth planning around rather than ignoring.
